Technical Assistance:
Discussed background checks, evacuation drills and handwashing.

Comments:
An unannounced monitoring inspection was conducted on 3/16/2022. At the time of entrance, 9 children were in care with 3 staff members present. Children were observed outside playing in the sand box, having snack on bench tables, listening to the teacher read a book and getting ready to go on a nature hike. Interactions between the children and staff were positive. A selection of staff and children records, the physical space, emergency supplies, evacuation drills and attendance records were reviewed. Standard #: 8VAC20-770-60-B
Description: Based on record review, the center did not have a sworn disclosure statement for each staff prior to date of hire.
Evidence:
1. During the inspection, the center did not provide Staff #2's sworn disclosure statement.

Plan of Correction: Center director will establish a checklist to ensure all documentation is completed
prior to being assigned to work with students. Staff #2 returns from vacation on
3/25/22. Director will have staff member sign required form.

Standard #: 8VAC20-770-60-C-2
Description: Based on review of two staff records, the center did not obtain the central registry finding within 30 days of employment.
Evidence:
1. Staff #1 (date of hire 1/01/2022) and Staff #2 (date of hire 1/28/2022) did not have the central registry findings at the center for their records.

Plan of Correction: Moving forward, center director will establish a system to ensure that a record is kept that
includes the dates forms are mailed and follow up communication to check on the results will
be printed and kept in the employees file. Employee 1 results are attached.
Employee 2 form was mailed on start date and email asking for results is attached.

Standard #: 8VAC20-780-160-A-1
Description: Based on review of two staff records, the center did not have documentation of a TB test or screening for one staff member at the time of employment and prior to coming into contact with children.
Evidence:
1. Staff #2 (Date of hire 1/28/2022) had a TB test or screening dated 2/28/2022.

Plan of Correction: Center director typed staff 2 start date incorrectly. Staff 2 start date was 2/28/22
(first day in classroom). TB test completed on 2/24/22, results received 2/28/22 prior to
engaging with children. Moving forward, center director will ensure
that TB testing is done prior to hire date moving forward.

Description: Based on observation, children's hands were not washed with soap and running water or disposable wipes before and after eating meals or snacks.
Evidence:
1. Nine students were observed having snack during the inspection and did not wash their hands with soap and running water or wipes before or after eating meals or snacks.

Plan of Correction: Center director has established a hand cleaning routine prior to and after
students eat.

Standard #: 8VAC20-780-550-D
Description: Based on interview, the center did not implement a monthly practice evacuation drill each month.
Evidence:
1. Staff A stated the center did not practice a monthly evacuation drill with the children in January or February 2022.

Plan of Correction: Center director has corrected the fire drill record to indicate need for
monthly vs quarterly fire drills. First fire drill completed on 3/21/22.

Standard #: 8VAC20-820-120-E-2
Description: Based on observation and interview, the findings of the most recent inspection of the facility was not posted.
Evidence:
1. Staff A stated the inspection summary from the initial inspection on 12/08/2021 was not posted.

Plan of Correction: Center director has posted the inspection summary from 3/16 and
updated records to indicate that summaries are to be posted moving
forward.
