Comments:
The Licensing Inspector conducted an in person investigation with another agency in response to a self-report incident of infant feeding on 3/15/2022. Interviews were conducted with staff and staff and child record reviewed. The current and new policy and procedures for infant feeding was reviewed. The infant classroom, refrigerator and infant bottles were inspected. The information gathered during this investigation supports the allegations; violations were discussed with the center director. Violations:
Standard #: 8VAC20-780-340-A
Description: Based on interviews, staff failed to ensure the care, protection and guidance for a 7 month old infant.
Evidence: On March 15, 2022, staff #1 failed to provide care and protection to child #1 when the child was fed the wrong bottle. According to staff #1, she picked up a bottle off the counter, warmed it and then fed about 3-4 ounces of breast milk to child #1. The breast milk bottle belonged to child #2, the bottle was properly label with child #2's name and was labeled "Breast Milk".
Staff #1 did not realize the error until staff #2 came back from lunch break and told staff #1 that she was feeding the wrong bottle to child #1.
Staff #1 admitted she failed to verify the label on the bottle before feeding child #2's bottle of breast milk to child #1.

Plan of Correction: All the infant staff were retrained on the center's policy and procedure for feeding infant. The bottles are already labeled with colored tape and a color tape chart is visible in the classroom. The center added another policy that staff must verify with the other staff in the classroom prior to administering any bottles. If the staff are alone, staff must call administration for verification prior to administering the bottle.
