22.1 Early Childhood Care and Education

Comments:
An unannounced renewal inspection was conducted today from 10:50am-12:25pm. There were 11 children (infants-2yrs.) directly supervised by 4 staff. The physical plant, 3 staff records, 3 children?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed eating lunch and napping. There was an adequate supply of materials for the children. Staff' MAT, CPR and First Aid certifications were verified, as well as DHO training. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice.

Violations:
Standard #: 22.1-289.035-B-4
Description: Based on record review, the provider did not obtain criminal history record results from a state in which the applicant has resided in the preceding five years before a final determination was made of the individual's eligibility to have responsibility for the safety and well-being of children.

Evidence:
Staff #1 was hired on 9/10/2021. Results of criminal history results from a state where the employee has resided within the previous five years were not obtained prior to employment. Staff#1 was present during the inspection.

Plan of Correction: That staff member was terminated the next day after the Licensing inspection, due to the fact that she was not following our directions and excessiveness in unexcused absences.
Last day of work was 3/11/2022.

Standard #: 8VAC20-780-160-A-2
Description: Based on record review, a staff's tuberculosis (TB) test/screening results were not completed within the last 30 calendar days of the date of employment.

Evidence:
Staff #2's (DOH: 2/28/2022) record contains a TB test/screening that was completed on 6/3/2021.

Plan of Correction: We sent the staff member to renew her TB test. The staff member won't be back until she brings it with her.

Standard #: 8VAC20-780-350-B
Description: Based on observation, staff-to-children ratios were not maintained in the Infant 1's classroom.

Evidence:
The Infant 1's classroom had 5 children with one staff member during the inspection. Upon checking ages of children in the classroom, it was determined that the youngest child is 3 months and the oldest child is 9 months, which requires the group to follow a ratio of 4 children to 1 staff member. An additional
staff member was assigned to the classroom shortly after the inspector entered the classroom.

Plan of Correction: When the inspector arrived, we had only 4 infants with 2 staff. Since I was busy with the inspector, second staff opened the door for 5th infant then immediately received the food form catering service delivery person. While she was dropping off the food in the kitchen, the inspector and I went inside the classroom. As the inspector noticed that, I immediately called her to join her classroom back.
We will make sure to always have the right staffing for the right age groups at all times like we always do.

Standard #: 8VAC20-780-570-A
Description: Based on observation, the protective belt was not securely fastened while an infant was sitting in an infant seat.

Evidence:
An infant was observed sitting in a Bumbo seat while eating a cracker, the Bumbo seat had a protective belt, but the child was not buckled into their seat by the caregiver.

Plan of Correction: We wrote up the teacher after reinforcing with her the importance of fastening infants while sitting in infants seats.
We have also reminded all our staff of the importance of strapping infants in the infants seats.
