Comments:
An unannounced inspections was conducted today. There were 9 children present with 6 staff supervising. The physical plant, children's records, staff records, menu's evacuation drills and first aid kits were reviewed. There were no medications on site. Description: Based on a review of children's records, the center failed to ensure that all required documentation was included in records for all children enrolled.

Evidence:
Child #3 and #4's records did not have documentation of a start date.

Description: Based on observation, the first aid kit did not have all required supplies.

Evidence:
The first aid kit did not have 2 triangular bandages
