A monitoring inspection was conducted on March 30, 2022 concerning a self reported staff supervision incident. Interviews were conducted and electronic data was reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-40-E Description: Based on interview with management, the licensee did not ensure the center's compliance with their own policies and procedures.
Evidence
Staff #1 did not follow the center's own policy and procedure of threshold counts and name-to-face check to ensure that all children are accounted for the the group.Plan of Correction: We will continue to follow VA Licensing
requirements/guidelines. We will also retrain staff on name-to-face procedure.
Standard #: 8VAC20-780-340-F Description: Based on interview with management, review of electronic information a child under 10 years of age was not always within actual sight and sound supervision of staff.
Evidence
Child #1 who is under the age of 10, was not always within actual sight and sound supervision of staff for approximately less than one minute.Plan of Correction: We will continue to follow VA Licensing requirements/guidelines. We will also retrain staff on name-to-face procedure.
