Standard #: 8VAC20-780-130-A Description: Based on the review of three children's records and verification from the director, there was not an immunization record in the file for one child showing that the child was adequately immunized.
Evidence
1. The record for Child #2 did not contain documentation that the child had received all of the immunizations required. Date of enrollment for Child #2 was 08/24/2021.
2. The director confirmed that the immunization record was not in the child's file.Plan of Correction: The director will reach out to the parent of the child and obtain documentation of the child's immunization record as soon as possible.
