Comments:
A monitoring inspection was initiated and concluded on April 5, 2022. At the onset of the inspection there were 11 children in care ages 2.5 - four years old with three staff. A walking tour of the entire facility inside and outside was completed. Medication, first aid kit and emergency equipment were reviewed. Violations regarding resilient surface and bottom covers on cots. These violations were discussed during the exit interview with the program director.

Violations:
Standard #: 8VAC20-780-245-J-3
Description: Based on interview the center failed to ensure that any child for whom emergency medications have been prescribed they will always be in the care of a staff member with Medication Administration Training (MAT).
Staff 1 and 2 stated that staff 2 does not have MAT and is responsible for transporting children to and from public schools. Three of children who are regularly transported require emergency medication.

Plan of Correction: Staff 2 will obtain the required training as soon as possible.

Standard #: 8VAC20-780-330-B
Description: Based on observation the center failed to maintain required resilient surface under around and in the fall/use zones as required.
Evidence:
There is approximately one to three inches of resilient surface under around and into the fall/use zone of two pieces of climb and slide equipment on the playground where six inches is required.

Plan of Correction: Center management will make arrangements to have the resilient surface corrected so that there is six inches of resilient surface under around and into the fall/use zone of all climb slide play equipment.

Standard #: 8VAC20-780-450-A
Description: Based on observation and interview the center failed to ensure that when children are napping there is bottom cover on cots.
Evidence:
1 - In the three year old room there were five children napping on cots without a bottom cover. In the four year old room there were four children napping on cots without a bottom cover.
2 - Staff present in both rooms confirmed that there were children napping without bottom covers on their cots.

Plan of Correction: Center management will remind and retrain staff to use the supply of center blankets when children do not bring their own top and bottom cover for nap time.
