22.1 Early Childhood Care and Education

Technical Assistance:
Technical Assistance was provided regarding:

-requirements for allergy care plans for children with food allergies
-requirements for child files and parent agreements
-requirements for staff files

Comments:
An unannounced monitoring inspection was conducted on 3/31/22 from 9:40am to 12:40pm with the Assistant Head of School. There were 54 children in care, ranging in age from 18-months to six-years-old, supervised by six staff. The children were observed serving themselves breakfast, playing musical instruments, practicing buttoning a jacket with help from the teacher, and playing quietly in different areas of the classrooms. Five child records and three staff records were reviewed. The Center has all staff certified in CPR and First Aid. Three medications and authorization forms were reviewed and the center has multiple staff current in Medication Administration Training (MAT). The attendance and emergency drill log were reviewed. The most recent Fire Inspection on file was dated 4/1/21 and the most recent Health Inspection on file was dated 3/29/22.

Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 4/6/22. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Standard #: 22.1-289.035-B-2
Description: Based on review of three staff records, the center did not obtain the results of a national fingerprint-based criminal history record check prior to employment for a staff member. Evidence: The results of a national fingerprint-based criminal history record check on record for Staff #3 were dated 12/14/20. Orientation for Staff #3 was dated 11/24/20.

Plan of Correction: New hire checklist will be updated to reflect hire date/start date requirements to resolve conflicts between the two.

Standard #: 8VAC20-770-60-B
Description: Based on review of three staff records, the center did not obtain documentation of a completed sworn disclosure statement for each staff prior to employment. Evidence: The sworn statement on record for Staff #3 was dated 11/30/20. The orientation training for Staff #3 was dated 11/24/20.

Plan of Correction: New hire checklist will be updated to reflect hire date/start date requirements to resolve conflicts between the two.

Standard #: 8VAC20-780-130-A
Description: Based on review of five child records and interview, the center did not obtain documentation that each child had received the immunizations required before attending the center. Evidence: The record for Child A, who was on site during the inspection, did not contain documentation of immunizations.

Plan of Correction: Mom has been emailed to provide the necessary documents

Standard #: 8VAC20-780-160-A-1
Description: Based on review of three staff records, the center did not ensure that documentation of tuberculosis (TB) screening was obtained and submitted by a staff member within the required timeframe. Evidence: Staff hired prior to 10/13/21 were required to submit results of a negative TB screening within 21 days after employment. The record for Staff #1, with a hire date of 6/1/21, contained a TB screening dated 9/8/21.

Plan of Correction: New hire checklist will be updated to reflect hire date/start date requirements to resolve conflicts between the two.

Standard #: 8VAC20-780-160-C
Description: Based on review of three staff records, the center did not ensure that a staff member obtained and submitted the results of a follow-up tuberculosis (TB) screening at least every two years from the date of the initial screening. Evidence: The most recent TB screening on record for Staff #3 was dated 2/6/20.

Plan of Correction: Renewal dates will be noted on calendar with automated reminders.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the center did not ensure that hazardous substances were kept in a locked place using a safe locking method that prevents access by children. Evidence: In the "Green House" classroom, with children aged three-years-old to six-years-old, there were four cleaners containing bleach on an upper open shelf in the bathroom.

Plan of Correction: The cleaning products were moved to a cabinet in the classroom and a lock was added to the cabinet during the inspection.
