Comments:
An unannounced renewal inspection was conducted on 4/06/2022. At the time of entrance, 66 children were in care with 7 staff members present. Children were observed in circle time reciting their ABC?s and counting 1-20, working on math worksheets, singing the finger song, a group activity with the easel pad, playing with playdough and playing a group game. The site was organized and contained developmentally appropriate materials. Interactions between the children and staff were positive. A selection of staff and children records, the physical space, evacuation drills, attendance records and medications and allergy action plans were reviewed. Description: Based on observation, staff and children's records were not treated confidentially.
Evidence:
1. The center's supplemental page for inspections (dated 3/23/2020, 9/15/2020, 1/12/2021, 9/21/2021, 10/14/2021, 10/15/2021) were posted on a wall in a clip board that states "Information found on the Supplemental Information page is confidential and this document is not to be posted in the facility."

Plan of Correction: 1. Postings were removed on 4/6/2022
that were deamed confidential and
placed in our Licensing and Subsidy
Binders.

Description: Based on observation, the areas and equipment of the center inside were not maintained in a safe and operable condition.
Evidence:
1. Paint was peeling in the following areas:
In the 1st Learning Adventure room-behind a desk in the classroom.
In the Preschool A classroom-under the window, near the green ABC board and bathroom.
In the 2nd Learning Adventures room-near the door.
In the Transitional Kindergarten classroom-near the library, near the door, near the cots and window.
In the Pre-K B classroom-in the bathroom, left of the door.
In the Pre-K A classroom-near the cots and right of cubbies and left of cubbies
In the Klubmates classroom, the entire room under the trim.
In the School Age classroom, near the door on both sides.
2. In the Transitional Kindergarten classroom, a children's red sofa had holes in both corners.
In the Pre-K B classroom, a children's blue sofa had a small hole in the middle.
In the Pre-K A classroom, a children's blue sofa had a small hole on the right arm

Plan of Correction: 1. Work orders have been placed for the entire
center in all of the classrooms, lobby, bathrooms,
and exterior doors/trim. A quote was sent to out
to our facilities team.
