22.1 Early Childhood Care and Education

Technical Assistance:
Provided consultation on:
1) written procedures for prevention of shaken baby syndrome or abusive head trauma
2) the center shall inform all staff who work with children of children's allergies, sensitivities and dietary restrictions
3) the center shall develop written playground safety procedures that shall include provision for active supervision by staff to include positioning of staff in strategic locations, scanning play activities and circulating among children

Comments:
An unannounced monitoring inspection was conducted from 11:30 am - 1:00 pm with staff. There were 32 children, ranging from nine months to five years of age, present with six staff supervising. Children were observed playing outside, washing hands, eating lunch and resting. Three children's records and three staff records were reviewed. One emergency medication, it's authorization form, the emergency drill log, the written list of allergies, and fire and health inspections were all reviewed.

Violations:
Standard #: 8VAC20-780-40-M
Description: Based on observation, and an interview with staff, the facility failed to maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions that is dated and kept confidential in each room or area where children are present. Evidence: there was no written list of allergies, sensitivities and dietary restrictions in the young children's community, the children's house, or on the playground with the children.

Plan of Correction: Staff have created a written list, placed it on the clip boards under the attendance sheet, and will ensure it is wherever children are present.

Standard #: 8VAC20-780-60-A
Description: Based on review of three child records, and an interview with staff, it was determined that not all records contained all required information. Evidence: the record for child #1, with a diagnosed food allergy, did not contain a written care plan to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Plan of Correction: Staff gave mom the form and requested that it be completed.

Standard #: 8VAC20-780-270-A
Description: Based on observation, it was determined that not all areas outside the facility were maintained in a safe condition. Evidence:
1) the fence surrounding the outdoor play area has chipped and peeling paint
2) the garden box's boards are separated leaving exposed, protruding nails/screws
3) the picnic table has splintered wood that is deteriorating in some locations
4) a tree stump is located next to the fence enabling a child to potentially climb up and over the fence
5) there are shelves on the young children's play area fence enabling a child to potentially climb up and over the fence
6) on the young children's play area, there is wire fencing over the garden area with exposed sharp ends and rusty nails protruding

Plan of Correction: Staff stated that the tree stump has been moved further from the fence, the garden will be removed, and the other items will all be addressed so the outdoor play areas are maintained in a safe condition.
