Comments:
An unannounced monitoring inspection was conducted on April 12, 2022. There were 19 children 5 staff. The physical plant, 3 staff records, 3 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed playing with board games, practicing writing, playing outside, sensory play, and washing hands in preparation for snack.. There was an assortment of age appropriate materials for the children in care. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Description: Based on observation, not all equipment was maintained in a safe condition.
Evidence:
The plastic lid to an outdoor bin was observed to be cracked and had exposed sharp edges.

Plan of Correction: The lid was removed and disposed of immediately.
