Comments:
A monitoring inspection was conducted on 03/31/2022 between the hours of approximately 11:00 am to 1:15 pm. There were 63 children present , with 8 staff supervising.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. The inspector measured two rooms that were recently added to the center. A total of 3 child records and 3 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-780-130-E
Description: Based on record review, the center did not obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence:
1) The most recent immunization record for Child #1 (dob 02/25/2021) was dated 04/28/2021 and for Child #2 (dob 02/17/2021) was dated 0/18/2021

Plan of Correction: Children's records were reviewed and parents were notified via email and verbally to provide updated immunization records.

Description: Based on record review, children's records did not contain all required information.
Evidence:
1) The records for Child #1, Child #2 and Child #3 did not contain the work phone number for each one of the parents who have custody.

Plan of Correction: Children records were reviewed and parents' phone numbers were updated.

Standard #: 8VAC20-780-60-A-8
Description: Based on record review, the center did not ensure to have a written care plan for each child with a diagnosed food allergy.
Evidence:
1) The records for Child #4 and Child #5 did not contain a written care plan to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. There is documentation in the records for Child #4 and Child #5 that identify food allergies and emergency medication on site for Child #4 and Child #5.

Plan of Correction: Allergy care plan was emailed to children's parents and will be put in the children's records.

Standard #: 8VAC20-780-70
Description: Based on record review, staff's records did not contain all required information.
Evidence:
1) The records for Staff #1, Staff #2 and Staff #3 did not contain name, address and telephone number of a person to be notified in an emergency.

Plan of Correction: Records for all the staff were updated to include required emergency contact information. New hire checklist is updated to check for this information for newly joining employees.

Standard #: 8VAC20-780-240-E
Description: Based on record review, one staff person did not complete orientation training in first aid and cardiopulmonary resuscitation (CPR).
Evidence:
1) The record for Staff #3 (start date 05/01/2021) did not contain documentation of having completed training in first aid and cardiopulmonary resuscitation (CPR).

Plan of Correction: First aid and CPR training for the employee will be scheduled and completed along with other new hires.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the center did not ensure that hazardous substances such as cleaning materials, are kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1) There were two bottles of drain cleaner (Drano) in the cabinet under the sink of the Toddler classroom. The cabinet door was not locked nor had installed a locking mechanism.

Plan of Correction: The bottles of drano are secured in a different locked cabinet on the same day. Staff educated to make sure it is always stored in a locked location.

Standard #: 8VAC20-780-510-P
Description: Based on medication and documentation review, medication with expired authorization forms were not returned to parents as required.
Evidence:
1) On the date of inspection, 3 different medications were kept in the center for Child #3. The authorization forms for these medication had an expiration date of 01/10/2021. Per standards, when an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days should be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet.

Plan of Correction: Expire medication was disposed off with the consent of child's parents.

Standard #: 8VAC20-780-560-G
Description: Based on observation, food brought from home was not clearly dated and labeled in a way that identifies the owner.
Evidence:
1) On the date of inspection, in the refrigerator in the Infant room there were two bottles with milk in them and a bottle of baby formula. These bottles had no date or name on them to identify the owner.

Plan of Correction: All the food items were labeled and dated immediately, staff was instructed to ensure it is done every time.
