Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and in person observations of the program.

A complaint inspection was initiated on March 29, 2022 continued in the center on April 4, 2022 and concluded on April 14, 2022. There were 3 children present on March 29, 2022, ranging in ages from 3 years to 4 years, with 1 staff supervising and on April 4, 2022 there were 5 children present ranging in ages from 7 months to 4 years, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, staffing and supervision, programming and special care and emergencies. A total of 7 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-210-A-4-a
Complaint related: Yes
Description: Based on record review and staff interviews, the licensee did not ensure that within six months before being promoted or beginning work or one month after being promoted or beginning work, a minimum of 24 hours of training shall be received related to the care of children when hired or promoted after June 1, 2008.

Evidence: The Center Director, Staff #1 confirmed Staff #3, Staff #4 and Staff #5 are designated as Program Leaders and do not meet the Program Leaders qualifications based on insufficient training/ education within the required timeframe. Staff #3 had 10 hours of training, Staff #4 had 8 hours of training and Staff #5 had 13 hours of training.

Plan of Correction: Staff completed outstanding trainings to satisfy this requirement. Director will ensure that future promotions abide by this regulation.

Standard #: 8VAC20-780-340-C
Complaint related: Yes
Description: Based on observation and staff interviews, the license did not ensure there always shall be on the premises when one or more children are present one staff member who meets the qualifications of a program leader or program director and an immediately available staff member, volunteer or other employee who is at least 16 years of age, with direct means for communication between the two of them.

Evidence: Upon Licensing Inspectors arrival on March 29, 2022, at approximately 7:30 pm one staff, Staff #1 was observed with three children (Child #1 age 3, Child #2 age 3, and Child #3 age 4). Staff #1 confirmed they were alone with children since approximately 6:10 pm after Staff #6 left the center.

Plan of Correction: Director/owner will ensure that there is at least one staff member who meets the qualifications of a program leader and / or a program director for one or more children. In addition, the second person will be at least 16 years old and able to effectively communicate to the program leader and /or director.

Description: Based on record review, observation and staff interviews, the licensee did not ensure that in each grouping of children there was at least one staff member who meets the qualifications of a program leader or program director.

Evidence: Upon Licensing Inspectors arrival on April 4, 2022 at approximately 6:50 pm, Staff #6 was observed as the only staff supervising the infant room and there were two infants in care. The Center Director, Staff #1 confirmed Staff #6 is designated as a Program Aide and does not meet the qualifications for program leader or program director.

Plan of Correction: Each classroom will have one staff who meets qualifications for program leader.

Description: Based on 0bservation and staff interviews, the licensee did not ensure that when any surface has been contaminated with body fluids, it shall be cleaned and sanitized.

Evidence: On April 4, 2022 a child was observed playing with a pink, plastic toy and putting it in their mouth. Staff #3 took the pink plastic toy, put hand sanitizer on a paper towel and wiped off the toy. The toy was not cleaned and sanitized after it was contaminated with bodily fluids.

Plan of Correction: Director and Team Leader will ensure that all staff have training / review on the appropriate way to sanitize toys and other items in order to ensure the safety and wellbeing of the children enrolled at GDC.
