Violations:
Standard #: 8VAC20-780-280-B
Description: Based on observation and interview, it was determined the center has not ensured hazardous substances (cleaning spray bottles) are being kept in a locked place using a safe locking method.
Evidence:
1. In the two toddler classes and the two's class, the changing station area keeps the disinfectant spray bottles up in the cabinet above the changing pad. These cabinets are not locked.

Plan of Correction: I have locks and will install them today.

Standard #: 8VAC20-780-550-P
Description: Based on record review of recorded injuries, it was determined the staff did not record what future action to prevent recurrence of the injury would be.

Plan of Correction: We will make sure we record actions taken to avoid recurrence.
