Comments:
A renewal inspection was initiated on April 21, 2022 and concluded on April 21, 2022 . There were a total of 25 children with 2 staff members present. Children were observed arriving to the morning program, during snack time, and engaged in group activities. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff training records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Description: Based on review of center policy and center supervisor interview, the center did not develop written procedures for abusive head trauma.

Evidence, after reviewing the center's procedures and interviewing the center supervisor, there was not a written procedure for abusive head trauma.

Plan of Correction: Center supervisor will request plan from administrative office.
