Comments:
The licensing inspector conducted an unannounced inspection in response to a self-reported concern regarding special services that occurred at the center on April 05, 2022. The Department was notified of the incident on April 7, 2022. The investigation was initiated on April 7, 2022 and concluded on April 14, 2022.

On April 14, 2022, the licensing inspector was on-site from 10:45am to approximately 12:05pm. Interviews were conducted, observations were made in the center, and documentation was reviewed. The preponderance of the evidence gathered during the investigation supported the self-reported incident. Any violations not related to the self-report but identified during the course of the investigation can be found on the violation notice.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Violations:
Standard #: 8VAC20-770-60-C-2
Description: Based on a review of staff records and interview, the center did not ensure that each staff record reviewed contained a central registry finding within 30 days of employment.
Evidence: The record of staff #1 (hired 11/29/21) contained documentation of a central registry finding dated 2/24/22. Administration acknowledged the result was late.

Plan of Correction: CD and AD will ensure proper paperwork is completed within appropriate time frame.

Standard #: 8VAC20-780-40-E
Description: Based on staff interviews and a review of the center policies, the center did not ensure they complied with their own policies and procedures that are required by these standards.
Evidence: 1.) On April 5, 2022, infant #1 (age 4 months) was fed approximately an ounce of infant #2's (age 7 months) breastmilk. Administration reported that staff #1 did not follow the center's policy on infant feeding, which is to call out the name on the bottle prior to giving the bottle to an infant. Administration was present in the infant room at the time of occurrence and witnessed that staff #1 did not call out the infant's name to match the name to the bottle. 2.) In an interview with staff #1, she reported that "I didn't do what I'm supposed to do in calling out the names." 3.) The center's infant care instructions state "when feeding an infant a bottle ensure you always verify you have the correct bottle to feed to the correct child."

Plan of Correction: Teachers will continue to be trained, retrained, and monitored on proper bottle care and use of "call out" system.

Standard #: 8VAC20-780-340-A
Description: Based on staff interviews, the center did not ensure when staff are supervising children, they shall always ensure their care and protection.
Evidence: 1.) In an interview with staff #1, she reported that on April 5, 2022, she fed infant #1 (age 4 months) approximately an ounce of infant #2's (age 7 months) breastmilk without checking if the bottle belonged to infant #1. She stated she removed both infant #1's and infant #2's bottles from the refrigerator and placed them in the bottle warmers. She then stated both bottles were removed from the warmers and placed on the counter. Staff #1 said she grabbed one of the bottles and fed it to infant #1 but did not check if the bottle belonged to infant #1.

Plan of Correction: Teachers were retrained very next day on proper bottle care. We went over & retrained again with CD in class on the "call out" system. Each teacher signed off and completed training. In class training for 5 days with each teacher.

Standard #: 8VAC20-780-500-A
Description: Based on observation and interview on April 14, 2022, the center did not ensure that staff and children's hands shall be washed with soap and running water after a diaper change.
Evidence: 1. A staff member was observed to change an infant's diaper. After the staff member changed the infant's diaper, the staff member didn't wash her hands with soap and water. The staff acknowledged she didn't wash her hands.

Plan of Correction: Retrain handwashing for all staff members. Diaper changes and bodily fluids.

Standard #: 8VAC20-780-570-E
Description: Based on observation and interview on April 14, 2022, the center did not ensure each infant bottle was labeled with the child's name and dated.
Evidence: In the infant room refrigerator there were three prepared bottles of formula not dated. A staff member acknowledged they weren't dated.

Plan of Correction: Retrain on checking bottle labels and making sure the correct information is listed on bottles.
