Inspector had a conversation with program owner regarding blankets in cribs as well as appropriate infant bib usage.

Today an unannounced Monitoring Inspection took place between the hours of 8:50am and 10:50am. There were 5 classrooms observed with a total of 31 children with 6 staff within the supervision guidelines. The children were observed washing their hands upon arrival, eating breakfast, receiving bottles, participating in circle time and other organized activities. A complete inspection of the physical plant, children and staff records, fire drill log and emergency procedures were observed during this inspection. Violations were found during today's inspection and can be viewed on the violation notice.

Standard #: 8VAC20-780-130-A Description: Based on record review and interview, the center has not obtained documentation that each child has received immunizations.
Evidence: Child #4's record does not contain documentation of immunizations. The program director stated that the child's parents stated the child has not received immunizations.Plan of Correction: Spoke with mom on 4/20 about shot records. She did provide them upon pick up.
Standard #: 8VAC20-780-140-A Description: Based on record review and interview, the center has not obtained documentation of a physical exam for each child within the first 30 days of attendance.
Evidence: Child #4 (date of enrollment: 2/22/2022) record does not contain a physical exam. The program director stated that the child's parents stated the child has not had a physical exam.Plan of Correction: Spoke with mom about needing a physical. It has been scheduled.
Standard #: 8VAC20-780-270-A Description: Based on observation, areas and equipment of the center were not maintained in a clean, safe and operable condition.
Evidence:
1. There is a soft green play structure in the infant classroom that has several rips and tears, exposing the internal foam structure.
2. 4 out of 7 bikes on the playground are missing the plastic handle bar covers.Plan of Correction: 1. Infant play structure was thrown out.
2. Handle bars are ordered. Will be delivered 4/25/2022.
