Discussed the radio and the requirement to have one that runs on batteries.

Comments:

An unannounced renewal inspection was conducted on 4/26/2022. At the time of entrance, 31 children were in care with 6 staff members present. Children were observed making snack bags and decorating them, making bird feeders with bagels and bird seed and watching a magic school bus movie on earth day. The site was organized and contained developmentally appropriate materials. Interactions between the children and staff were positive. A selection of staff and children records, the physical space, evacuation drills and attendance records were reviewed. Areas of non-compliance are identified in the violation notice. Description:	Based on record review, the center did not obtain a copy of the results of a sex offender registry check or criminal history record for any state in which the individual has resided in the preceding five years within required timeframe. Evidence: 1. Staff #1 (Date of hire 8/11/2017) lived in another state (Texas) in the last 5 years and had a (TX) sex offender check dated 3/02/2022 and (TX) criminal record check dated 3/02/2022. This requirement was due by 12/31/2020.

Description:	Based on observation, staff and children's records were not treated confidentially. Evidence: 1. The supplemental page dated 7/13/2021 was posted on the wall and is considered confidential.

Description:	Based on observation, the areas and equipment of the center inside were not maintained in a safe and operable condition. Evidence: 1. In the sunshine classroom, a yellow wooden chair was chipping paint. A floor rug's edge was fraying and coming apart in an area approximately 15 inches. 2. In the Rainbow classroom, there was 3 cracked tiles in the bathroom on the wall. 3. Two stakes were protruding up on the playground near some rotten wood.
Plan of Correction:	Chair and floor rug will be repainted or replaced; work orders placed for broken tiles and stakes to be repaired.

Description:	Based on interview and observation, the center did not have one working, battery-operated radio in each building on each floor of each building as required. Evidence: 1. The center had a radio, but it was not battery operated.
