Comments:
An unannounced monitoring inspection was conducted today. There were 11 children present with 2 staff supervising. The children were observed handwashing, playing in activity areas, singing, playing outside on the playground and eating snack. The physical plant, children's records, staff records, evacuation drills, the first aid kit, emergency preparedness plan and playground safety plan were reviewed. Standard #: 8VAC20-780-60-A
Description: Based on a review of children's records, the center failed to obtain required documents for all children enrolled.

Evidence:
Child #1's record did not contain documentation of work phone numbers for parents.
Child #2's record did not contain documentation of work phone numbers for parents
Child #4's record did not contain documentation of work phone number and place of employment for each parent nor did it contain documentation of names of persons authorized to pick up the child.
Child #5's record did not contain documentation of a work phone number for mother of child.

Plan of Correction: I will contact parents and request the information.

Standard #: 8VAC20-780-80-A
Description: Based on a review of documents, the center failed to maintain a written record of daily attendance that documents the arrival and departure for each child in care as it occurs.

Evidence:
The center maintains a record of attendance, but did not have a record as to the departure of children in care.

Plan of Correction: I will change the attendance form so that it can reflect when children departed center.

Standard #: 8VAC20-780-245-A
Description: Based on review of staff records, the center failed to ensure that each staff member will obtain 16 hours of training annually.

Evidence:
Staff #1, #2 and #3's records did not have documentation of training for 2021-2022 school year.

Plan of Correction: We will complete training as soon as possible.

Standard #: 8VAC20-780-560-G
Description: Based on observation of lunch boxes, the center failed to ensure that food brought from home shall be clearly dated and labeled in a way that identifies the owner.

Plan of Correction: Staff will label and date food brought from home, that has not been labeled by parents.
