Technical Assistance:
Discussed medication authorization forms, allergy action plans, carbonmonoxide detector requirement and maintence of the resilent surfacing.

Recommended director and staff to review new guidance and model forms on the public website https://www.doe.virginia.gov/cc/providers/index.html?pageID=10.

Recommend to review Code ? 22.1-289.058. Child day programs -Each building that was built before 2015 and that houses a child day program that is licensed that serves preschool-age children shall be equipped with at least one carbon monoxide detector.

Comments:
An unannounced monitoring inspection was conducted on April 12, 2022 from approximately 10:05 am-12:45pm. There were 57 children present at this time with 10 staff members. Additional staff were on site for administrative and support services. Children were observed participating in station activities, dancing , singing and tummy time . Three staff and four children's records were reviewed during the inspection. Licensing Inspector reviewed indoor areas, supervision, activities, equipment, medication procedures, emergency evacuation and shelter-in-place drill documentation, emergency procedures and required posting.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-140-A
Description: Based on record review, the licensee did not ensure there was a physical obtained before the child attendance or with 30days of start date.

Evidence:
Child #2 (start date 12/2/2019) did not have a physical with in 30days of start date.

Plan of Correction: Parent contacted and physical recieved .

Standard #: 8VAC20-780-270-A
Description: Based on observation , the licensee did not ensure the equipment outside is maintained in clean, safe and operable condition.

Evidence:
On the playground area, there wer 2 of 4 toy lawn mower were broken with sharp points on the side, 2 of 2 radio flyers had baskets were broken with sharp points, 1 of 4 rocking horses had a broken handle that had sharp point and pinch points.

Observations were made with the director on the playground.

Plan of Correction: Playground has been cleared of any broken toys.

Description: Based on observation , the licensee did not ensure subsitute containers indicate the content.

Evidence: In the the Wobbler room, there was a hand soap in a bottle labeled hand sanitizer. The center Director confirmed it was hand soap not hand sanitizer.

Plan of Correction: Bottle have been properly labeled .

Description: Based on observation, the licensee did not enure dipaering area is non-absorbant and able to be cleaned and santized after each use.

Evidence: In the Preschool classroom, there was multiple tears on the diapering surface.

Plan of Correction: Changing pad replaced .

Standard #: 8VAC20-780-540-E
Description: Based on observation and staff interview, the licensee did not ensure there was a working battery operated radio.

Evidence : Center director confirmed there was not a working battery operated radio.

Evidence:
Director confirmed the center did not have working battery operated radio.

Plan of Correction: Placed batteried in the radio.

Standard #: 8VAC20-780-550-C
Description: Based on observation and staff interview, the licensee did not ensure there was a emergency evactuation and shelter in place map or procedures posted in the building.

Evidence: The shelter in place map or procedures were not posted in the building. Confirmed with the Director.

Plan of Correction: All maps posted in all rooms.

Standard #: 8VAC20-780-550-P
Description: Based on record review, the licensee did not ensure all required written record of children's injuries contained all required information.

Evidence: Injury records reviewed, 5 of 6 injury records did not have the time the parent was notified. Confirmed with the program director.

Plan of Correction: Staff trained to mark parent time and notification.
