Comments:
A monitoring inspection was initiated on Monday, April 25, 2022 and concluded on Friday, April 29, 2022. There were 37 children present with 8 staff supervising and 2 administrative staff during the in person observation by the inspector at the center. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-780-160-A-2
Description: Based on staff record review and interview, the center did not ensure to obtain the TB documentation within the last 30 calendar days of the date of employment.

Evidence: In Staff#1's record (date of hire: 3/30/22), the documentation was TB was dated 09/16/21.
Interview with staff confirmed the TB provided is the most recent available during this inspection.

Plan of Correction: The Director will ensure documentation for staff TB is within 30 calendar days as required.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the center did not ensure all hazardous substances are kept in a locked place using a safe locking method that prevents access by children.

Evidence: 1. In the Ones classroom, there was an unlocked storage closet and inside there was cabinet located on the floor observed unlocked containing various cleaning and disinfectant products with the labels "warning" and "keep out of reach of child".

Plan of Correction: The Director redirected staff to keep all hazardous substance locked at all times. On site inspections were completed throughout the week to verify compliance by the Director.
