22.1 Early Childhood Care and Education

Comments:
An unannounced monitoring inspection was conducted today from 2:00pm-3:15pm. There were 15 children directly supervised by 2 staff. The physical plant, 2 staff records, 2 children?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed napping, completing worksheets, working on Montessori based activities and practicing the alphabet. There was an adequate supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice.

Standard #: 22.1-289.035-B-4
Description: Based on record review, the center did not obtain criminal history record results, abuse and neglect results and a sex offender registry check from a state in which the applicant has resided in the preceding five years before a final determination was made of the individual's eligibility to have responsibility for the safety and well-being of children.

Evidence:
1. Staff #1's (DOD:9/7/2021) record did not contain results of a criminal history result and a sex offender check from a state where the employee has resided within the previous five years. Results were required prior to employment.
2. Staff #1's record did not include abuse and neglect results from a state where employee has resided within the previous five years, abuse an neglect results are required within 45 days of employment.

Plan of Correction: Prior to the staff's first work day, if they mentioned on their application that they lived in another state within the last 5 years. We are responsible for looking their name up in the state sex registry, obtain a criminal history for that state and abuse and neglect record.
In this case we sent forms out and are awaiting results, the staff's name in the sex registry came back clean.

Description: Based on record review, the center did not obtain central registry results within 30 days of employment for each staff member.

Evidence:
Staff #1 (DOH : 9/7/2021) and Staff #2's ( DOH: 9/7/2021) records did not contain Central Registry results.

Plan of Correction: Obtained the correct form online and had them completed, notarized and we sent the form to the address listed.

Description: Based on review of documentation and interview, the center did not develop written procedures for prevention of abusive head trauma.

Evidence:
There was no documentation at center to verify that the center has a written procedures for prevention of abusive head trauma.

Plan of Correction: We came up with a written procedure for AHT and will update on a as needed basis.

Description: Based on documentation review and interview, the center did not develop and implement a written policy and procedure that describes how the center will ensure that each group of children receives care by consistent staff or team of staff members.

Evidence:
There was no documentation at the center to verify that they have a written policy and procedures that describe how the center will ensure that each group of children receives care by consistent staff or team of staff members.

Plan of Correction: A policy and procedure was created for:
1) Illnesses and communicable diseases.
2) When a child is sick at school
3) In case of a serious injury or illness

Standard #: 8VAC20-780-540-C
Description: Based on observation, the center's first aid kit was incomplete.

Evidence:
The first aid kit was missing adhesive tape.

Plan of Correction: Adhesive tape was placed in first aid kit. Kit will be monitored closely and supplies will be stocked regularly.

Standard #: 8VAC20-780-550-D
Description: Based on review of emergency drills, evacuation drills were not practiced monthly.

Evidence:
There was no documentation to verify that an evacuation drill was practiced in September 2021.

Plan of Correction: At the start of the school year, we will make it our duty to have a monthly evacuation drill.

Description: Based on observation and interview, the findings of the most recent inspection of the facility were not posted.

Evidence:
The Violation Notice issued on 9/30/2021 was not posted.

Plan of Correction: The newest inspection report is posted on the board and will be updated and posted as we get them.
