Comments:
The licensing inspector conducted an unannounced monitoring inspection, at which play groups, outside time, group time, and lunch time were observed. Children were observed; and staff and children files and other documentation were reviewed. Seventy-nine (79) children were found to be in care. Please contact me if you need further assistance. Thank you for your cooperation.

Standard #: 8VAC20-780-160-A
Description: Based on review of staff files and discussion with administration the facility failed to maintain compliance with this standard as required relating to PPD tests/screening.

Evidence:
Staff #1, hire date 1-18-22, did not have a PPD/TB test or screening.
Staff #2, hire date 1-10-22, did not have a PPD/TB test or screening.
Staff #3, hire date 11-15-21, did not have a PPD/TB test or screening.
PPD/TB tests or screenings are required to be completed prior to hire.

Plan of Correction: Staff will obtain a test or screening as soon as possible. In the future staff will not start work without the required test or screening to ensure compliance.
