Comments:
A monitoring inspection was conducted on 04/28/2022. There were 13 children present, ranging in ages from 5 to 12 years, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Description: Based on documentation review and interview with staff, an annual approval from the health department not provided,
Evidence:
1) On the date of inspection, the results of the health department's annual inspection were not available for review. 2) Staff #1 inquired with the school administration but a copy of the report was not available.

Plan of Correction: Inspector report submitted to the Licensing Inspector on 05/06/2022.
