Comments:
A monitoring inspection was conducted on 05/04/2022, from 9:30am to 12:07pm. There were 31 children present and 10 staff. The Inspector reviewed five children?s records and three staff records. The Inspector observed staff and children?s interactions throughout the inspection. Children were observed during free play both indoor and outdoor, circle time, toileting, meal preparation, and naptime. Medications, first aid kits, and emergency preparedness documents were observed. Transportation vans were viewed for compliance. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented. The violations are listed on the violation notice issued to the center and were reviewed with the program director and assistant director at the exit interview.

Violations:
Standard #: 8VAC20-780-270-A
Description: During observation and interview, it was the determined that the center did not ensure that areas and equipment of the center be maintained in a safe condition.

Evidence:

1) The entrance to the preschool room has approximately five inches of frayed and missing carpet, exposing the metal lining between the carpet and laminate. Children and staff were observed using this entrance during the time of the inspection. This poses a tripping hazard.
2) The toddler changing table has several plastic bags in reach of children posing a suffocation hazard.
3) Staff #1 confirmed that the entrance to the preschool room was currently being used to enter and exit the preschool area and that the area was in need of repair. Staff #1 was aware the plastic bags were in reach of children posing a suffocation hazard.

Plan of Correction: The center responded with the following: Staff #1 placed electric tape on top of the missing carpet and exposed metal during the inspection. The center will continue to observe the area until replaced. The center will be re-flooring the entire preschool floor with laminate during Memorial Day weekend 2022. The plastic bags were immediately removed and placed out of reach of children during the inspection.

Standard #: 8VAC20-780-280-B
Description: During observation and interview, it was determined that the center did not ensure that hazardous substances such as cleaning materials were locked in a place using a safe locking method that prevents access by children.

Evidence:

1) The infant room has two small open buckets with liquid cleaning solution beside the sink. The cleaning solution is in reach of the seven infants currently in care.
2) The infant room has an unlocked bottle of cleaning solution beside the changing table accessible to children.
3) The preschool bathroom has a bottle of cleaning solution unlocked on top of the first aid kit box accessible to children.
4) The toddler changing table had a bottle of bleach and water unlocked in reach of children.
5) Staff #1 confirmed that the above cleaning solution all contained a hazardous cleaning material. Staff #1 confirmed the cleaning solutions were unlocked in reach of children.

Plan of Correction: The center responded with the following: Assistant Director, Staff #1, immediately corrected the concern during the inspection by locking all required hazardous substances. Moving forward, the center will ensure all cleaning materials are locked.

Standard #: 8VAC20-780-280-G
Description: During observation and interview, it was determined that the center did not ensure that hazardous substances that are not kept in original containers are clearly indicate their contents.

Evidence:

1) The infant room has a bucket of cleaning solution beside the sink that is not labeled with its content and not in the original container.
2) The preschool bathroom has a bottle of cleaning solution, not in its original container, on top of the first aid kit box with no label of its content.
3) Staff #1 confirmed the cleaning bottles of bleach and water and the disinfectant solution did not have the required label to distinguish its content.

Plan of Correction: The center responded with the following: The Assistant Director, Staff #1, will label all required hazardous substances that are not kept in the original container.

Standard #: 8VAC20-780-330-B
Description: During observation and interview, it was determined that the center did not ensure that the playground equipment is provided with resilient surfacing under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles.

Evidence:
1) The slide to the preschool playground was placed on the grass and dirt area which does not contain resilient surfacing to the fall zone. Children were observed during playtime utilizing the slide.
2) The slide to the toddler playground has no resilient surfacing to the fall zone.
3) Staff #1 confirmed that there was no resilient surfacing as required under the slides on both playgrounds.

Plan of Correction: The center responded with the following: The center will place the required resilient surfacing to the bottom of both slides.

Standard #: 8VAC20-780-440-G
Description: During observation and interview, it was determined that the center did not ensure that cribs be used for children under 12 months of age.

Evidence:

1) The infant room has two children (age 6 months and 7 months) that are not sleeping in cribs.
2) Staff #1 and infant room teachers confirmed that both children are sleeping outside of a crib on mats on the floor of the classroom.

Plan of Correction: The center responded to the following: The two infants will sleep in their own designated cribs. Moving forward, all infants under 12 months will sleep in a crib.
