Technical Assistance:
The Virginia Department of Education (VDOE) assumed oversight of child care licensing and monitoring on July 1, 2021. All associated regulations and codes have transferred over from the Virginia Department of Social Services.

Comments:
An on-site renewal study was conducted today. 23 school-age children were present with 3 staff. Reviewed one new staff record, 4 children's records, staff trainings, written policies and procedures, emergency updates, medication and physical space. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 22.1-289.058
Description: Based on staff statement and observation, the program did not have a working carbon monoxide detector.
Evidence:
No carbon monoxide detector was available during the inspection.

Plan of Correction: A new one was purchased and installed in the Extended Day room.

Standard #: 8VAC20-780-60-A
Description: Based on record review, 2 of 4 children's records were incomplete.
Evidence:
Child #1 was missing the second page of the child record with parent signature, emergency authorization, and parent agreement.
Child #2 was missing allergy documentation, doctor information with phone number and a second emergency contact address.

Plan of Correction: Not available online. Contact Inspector for more information.

Standard #: 8VAC20-780-240-I
Description: Based on record review, staff #1 did not have orientation documentation that covered updated information on the Center standards as of 1/10/2022.
Evidence:
Orientation documentation was missing training documentation on abusive head trauma, consistency of care, allergy/intolerance and food preferences. Documentation was completed 1/24/2022

Plan of Correction: An updated orientation form was completed 2/16/2022 and 5/4/2022.

Standard #: 8VAC20-780-550-P
Description: Based on observation, the written injury report was missing information as required by the standards.
Evidence:
5 injury reports were reviewed-4 were missing the time the parent was notified.

Plan of Correction: Moving forward, injury reports will be reviewed and completed with all information.
