Comments:
A self-reported incident was received by the department on 4/19/2022 regarding roughly handling a toddler. An investigation conducted jointly with a local agency was initiated on 4/28/2022 and concluded on 5/02/2022. The director was contacted by telephone to begin the investigation. The inspector emailed the director a list of documentation required, the center's video recordings were reviewed and staff were interviewed.
Information gathered during the investigation of the self-report determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-410-1
Description: Based on interviews and observation of the center's video recordings, a staff member used a forbidden action of rough handling a child in care.
Evidence:
1. On 4/14/2022 at approximately 5:00pm, while Child #1 (18 months old) walked near a table in the Toddler classroom where a few of the children were sitting for snack, Staff #1 roughly grabbed Child #1?s left upper arm with her right hand, pulled out a chair at the table, picked Child #1 up off the floor by his left wrist and his right upper arm, roughly put the child in the chair and quickly slid the chair with the child up to the table.
2. While watching the center's video recordings of the incident, Staff #1 stated the following: "Made it seem I was rough.? ?It was a fast grab because of my body language, it looked rough.? ?I was wrong in the way I grabbed him."
3. Staff #2 stated "it looked a little aggressive." "I felt like, don't grab him by the arm and pick him up."
4. Staff #3 stated "be careful" to Staff #1 after she witnessed Staff #1 grab Child #1 by the arm, pull him and sit him down. Staff #3 stated it was ?inappropriate?, ?too rough? and was ?afraid [Staff #1] was going to hurt the child.?
5. Adult #1 witnessed the incident and wrote in an email to center management, Staff #1 ??forcefully grabbed him by the arm and roughly set him down in a chair.?

Plan of Correction: Teacher has a training on May 18th "What are you talking about (Emotionally Support Interaction). We are getting training support from Infant and Toddler. All support will be done June 10th.
