An unannounced monitoring inspection was conducted with the director on April 22, 2022 beginning at 12:20 PM and ending at 1:39 PM. There were 28 children, ages 8 months through 5 years old, in the care of 4 staff. Five children?s records were reviewed. Three staff records were reviewed and in compliance. There were no medications on site to be reviewed. The first aid kit, flashlight, and weather-band radio were observed and in compliance. The most recent evacuation drill was documented as 4/8/2022. The most recent shelter-in-place was documented 3/7/2022. The fire and health inspections were reviewed and in compliance.

Standard #: 8VAC20-780-130-A Description: Based on a review of documentation, not all children had documentation of immunizations in their record. Evidence: Child A was missing documentation of immunization. Plan of Correction: The director contacted the parent and requested a copy of the immunization records for the child.
