An announced Subsidy Health and Safety inspection was completed in response to a planned relocation of the licensed program. The inspection was conducted to review supplemental health and safety requirements.
A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: 8VAC20-790-600-F Description: Based on record review and interview, the vendor did not ensure that each staff complete annually the Virginia Annual Health and Safety Update Course sponsored by the Department of Social Services as required.
Evidence:
1. The record of staff #1 (DOH:8/26/2019) did not contain documentation of the Virginia Annual Health and Safety Update.
2. The vendor acknowledged that the training update was not completed by the staff annually as required.Plan of Correction: Staff #1 has begun the training
and will submit the certificate
immediately upon completion to
the licensing agent.
