Comments:
A renewal inspection was initiated on March 7, 2022 and concluded on March 8, 2022. There were 44 children present, ranging in ages from toddler to five years, with 10 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and four staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Description: Based on interview, the provider did not have a carbon monoxide detector and serves preschool-age children.

The administrator stated the center did not have a carbon monoxide detector.

Description: Based on observation, the center did not ensure all hazardous substances such as cleaning materials, are kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. In the closet located in classroom #1, Glade air freshener labeled ?Caution? and ?Keep Out of Reach of Children?; Microban sanitizing spray labeled ?Caution? and ?Keep Out of Reach of Children?; Clorox Wipes labeled ?Caution? and ?Keep Out of Reach of Children? was not locked to prevent access by children. In a cabinet in classroom #2, one container of Lysol Spray labeled ?Caution? and ?Keep Out of Reach of Children? was not locked to prevent access by children. On top of a cabinet in classroom #5, Microban sanitizing spray labeled ?Caution? and ?Keep Out of Reach of Children? was not locked to prevent access by children.
2. On bus #1, two containers of Lysol labeled ?Caution? and ?Keep Out of Reach of Children? were unlocked in the front of the bus. On bus #2, two containers of Lysol labeled ?Caution? and ?Keep Out of Reach of Children? were unlocked in the front of the bus.

Description: Based on observation, the center did not ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards.

Evidence:
On the playground, the climbing equipment had approximately 2 to 3 inches of depth in sand at the fall zone areas.

Description: Based on observation and staff interviews, the licensee did not ensure medication, except for those prescriptions designated otherwise by written physician's orders, including refrigerated medication and staff's personal medication, were kept in a locked place using a safe locking method that prevents access by children.

Evidence:
In classroom #1, an asthma inhaler was located in a container on top of the children?s cubbies. The container was not locked and was accessible to children.

Description: Based on observation, children's food items were not clearly dated and labeled in a way that identifies the owner.
