Technical Assistance:
Provided clarification regarding training on cpr and first aid versus obtaining certification.

Walked thru one unused classroom and provided consultation regarding the need for a diapering surface since children under the age of 3 who may not toilet trained will occupy this classroom.

Clarified the need to retain all health exams and immunizations that are obtained at the time of enrollment.

Comments:
An unannounced Renewal inspection was conducted this date. A total of 114 children with 22 staff were onsite. Observation of Classrooms # 1, 2, 8, 11 and 12 was made. Two classrooms, #10 and #9 are used for private school and are not included in licensing oversight. All 3 playgrounds were observed. Some children were observed playing on the toddler playground with staff positioned strategically to ensure adequate supervision. Children inside the classrooms were observed completing morning program activities and washing hands and preparing for lunch. Medication was found onsite and reviewed. Standard #: 8VAC20-780-240-E
Description: Based on staff record review, orientation training in first aid and CPR was not completed within 30 days of hire.
Evidence:
Staff #1,2 and 3 have not completed orientation training in first and CPR.

Plan of Correction: All new staff hired since October 13, 2021 will have orientation training on first aid and cpr. All new staff hired ongoing will complete an orientation on first aid and cpr.

Standard #: 8VAC20-780-270-A
Description: Based on observation of the center, two areas were not maintained in a safe condition.
Evidence:
1. One fence plank within reach of children was cracked and splintered creating a hazard.
2. The area carpet in Class #1 was curled on the corner of one side creating a tripping hazard.

Plan of Correction: Center Director will submit a work ticket for maintenance to address the needed repairs in these areas.

Standard #: 8VAC20-780-330-B
Description: Based on observation of the playgrounds, resilient surfacing did not comply with the minimum safety standards.
Evidence:
The resilient surfacing located around and under the climbing apparatus located on the toddler playground did not have a minimum depth of six inches of resilient surfacing.

Plan of Correction: Center Director will place a work ticket for additional mulch for the playgrounds.

Description: Based on review of medications, one medication was not properly labeled.
Evidence:
A liquid antihistamine in Class #8 was not labeled with a child's name.

Plan of Correction: This medication will be labeled with the child's name.

Standard #: 8VAC20-780-550-P
Description: Based on review of the written record of minor injuries, the record was incomplete.
Evidence:
1. The incident records dated 5/10/22 and 5/13/22 did not include the time the parent was contacted.
2. The incident record dated 5/12/22 did not include the time the incident occurred and the time the parent was contacted.

Plan of Correction: We will remind staff to complete all incident reports entirely to include the date and time the parent was notified and the date and time of the incident.
