Comments:
A complaint investigation was initiated on 10/27/21 and concluded on 05/05/22 in response to two complaints that were received by the department on 10/13/21 and 10/19/21 in the areas of the supervision of children, special care provisions, and administration. The inspector conducted an unannounced inspection on-site on 10/27/21 from approximately 9:30am to 2:10pm. Interviews with staff were completed, documentation was reviewed, and observations were made at the center. The preponderance of the evidence gathered during the investigation supports part of the allegation regarding staffing and supervision of children; therefore the complaint is determined to be valid. Complaint related: Yes
Description: Based on information obtained during interviews and documentation reviewed, the center did not maintain the required ratios for children birth up to 16 months of one staff member for every four children.

Evidence: 1) The Infants classroom Child Supervision Record (CSR) for 09/08/21, 10/13/21, 10/19/21, and 10/27/21, were reviewed. The following was determined:

a) On 09/08/21, there were six children and one staff member present at 8:45am. The fifth child arrived at 8:21am and the sixth child arrived at 8:25am. Two additional staff members were documented as being present at approximately 10:30am.

b) On 09/08/21, there were 10 children and two staff members present at 3:35pm. One additional staff member was documented as being present at approximately 4:30pm.
c) On 10/13/21, there were five children and one staff member present at 8:00am. A second staff member was documented as being present at approximately 9:15am.

2) Staff interviewed on 10/27/21 confirmed the Infants classroom is often out of ratios in the mornings and in the afternoons. The staff-to child ratios for children birth up to 16 months is one staff member for every four children.

Plan of Correction: Per the Center: "Adjustment of teachers schedules to account for early arrival of children. Hiring teachers/floaters to cover classrooms of teachers that have called out."

Description: Based on information obtained during interviews and documentation reviewed, the center did not maintain the required ratios for children 16 months old to two years of one staff member
for every five children.

Evidence: 1) The Toddlers classroom Child Supervision Record (CSR) for 09/08/21, 10/13/21,
10/19/21, and 10/27/21, were reviewed. The following was determined:

a) On 10/13/21, there were 15 children present and one staff member from 9:00am until 9:30am when a second staff member arrived. The classroom remained out-of-ratio until approximately 11am when five children were moved to another classroom.

c) On 10/19/21, there were six children present and one staff member from approximately
2:00pm until 4:18pm.

2) Staff interviewed on 10/27/21 confirmed the Toddlers classroom is often out of ratios in the
mornings and in the afternoons. The staff-to child ratios for children 16 months old to two years is one staff member for every five children.

Plan of Correction: Per the Center: "Adjustment of teachers schedules to account for early arrival of children. Hiring teachers/floaters to cover classrooms of teachers that call out"

Complaint related: Yes
Description: Based on observations, documentation reviewed, and interviews, the center did not maintain the staff-to-child ratios for children two years of age of one staff member to eight children whenever children are in care.

Evidence: 1) At approximately 9:38am, the licensing inspector observed nine children (2 years old) and one staff in the DPS 1 classroom and 11 children (2-3 years old) and one staff in the DPS 2 classroom. 2) A member of management acknowledged there were a total of 20
children present and only two staff members. A third staff member was asked to assist and
arrived in the classroom at approximately 9:45am. A fourth staff member arrived at 10:00am. 3) The staff interviewed reported the DPS 1 and DPS 2 classrooms are out of ratio on a regular basis. The staff-to-child ratios for children two years of age is one staff member to
eight children.

Plan of Correction: Per the Center: "Adjust teacher schedules to account for late student pick up. Hire new teachers to cover classrooms."
