Comments:
An unannounced monitoring inspection was conducted today from 11:00am-12:30pm. There were 40 children directly supervised by 9 staff and all classrooms were in ratio. The physical plant, outdoor playground area, programming, menus, 3 staff records, 5 children?s records, emergency drills, injury reports, emergency supplies, and policies were inspected. Children were observed eating lunch which consisted of cheeseburgers, fruit, corn, and milk, transitioning to naptime, and singing songs. Infant diapering and handwashing was observed and found to be in compliance. Positive interactions between children and staff were observed during this inspection. The center advised no medication was on site at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center did not ensure that areas inside the center are maintained in a clean and safe condition.

Evidence:
1. In the 2's bathroom between the two toilets, the cove base has detached from the wall and was observed laying on the floor revealing chipped paint and crumbling drywall.
2. In the 3's room, a hole in the drywall approximately 6" in size was observed just above the cove base and to the left of the doorway exiting the classroom.
3. In the Preschool room, to the left when entering the classroom along the numbers wall and between the toy shelves, the cove base was observed detached from the wall revealing crumbling drywall.

Description: Based on observation, the center did not ensure that hazardous substances, such as cleaning materials, are kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. In the Toddlers room, multiple bottles of cleaning solution and degreasing cleaners were observed in the unlocked cabinet beneath the sink in the classroom.
2. In the Toddlers unlocked bathroom, multiple bottles of bleach were observed on the floor of the bathroom. In addition, bottles of bleach and other cleaning supplies were located on shelves in the unlocked bathroom.

Description: Based on observation, the center did not ensure that a nonabsorbent surface for diapering or changing is used.

Evidence: A small tear, approximately 1" in size, was observed on the changing pad in the Infants room revealing the foam filling making the surface absorbent.
