Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and interviews.
A monitoring inspection was initiated on April 11, 2022 and concluded on May 19, 2022. The director was contacted by telephone and an in person inspection was conducted. The inspector reviewed compliance in the areas of administration, staff qualifications /training and staffing and supervision.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-185-340-A
Description: Based on a review of the center's documentation and interviews, the licensee did not ensure when staff are supervising children, they shall always ensure their care, protection, and guidance.
Evidence: On April 8, 2022, Child #1, age 3 was left unsupervised on the fenced in playground between approximately 12:04 pm and 12:11 pm. Center Director confirmed Staff #1 and Staff # 2 did not know that Child #1 was left unsupervised on the playground until the Center Director brought it to their attention.

Plan of Correction: The lead teacher responsible for that group of children was terminated April 8, 2022, following the situation. The assistant teacher for that group attended the training and has a written warning in their file.

Standard #: 22VAC40-185-340-F
Description: Based on interviews with staff and other parties involved, the licensee failed to ensure children under 10 years of age are within actual sight and sound supervision of staff.
Evidence: On April 8, 2022, Child #1, age 3 was left unsupervised on the fenced in playground between approximately 12:04 pm and 12:11 pm.
1) Video recording shows at approximately 12:04 pm, Child #1?s class came inside from the playground by going through the playground back door and walking through the large multipurpose room and then heading to their classroom. 2) Video recording shows a church volunteer, Volunteer #3 working in the multipurpose room and at approximately 12:11 pm, Child #1 opens the playground back door on their own and enters the multipurpose room. Child #1 asked Volunteer #3 ?where did everybody go?? 3) Volunteer #1 confirmed Child #1 was not in distress, crying or upset.

Plan of Correction: All staff were retrained on April 11, 2022 on staffing and supervision for transitions. Training was conducted in person and included staff proving their competence in following the procedures.
