Comments:
An unannounced monitoring inspection was conducted on 5/19/22 from 1:30pm to 3:00pm with the Owner and Director. There were 97 children in care, ranging in age from nine-months to five-years-old, supervised by eight staff. The children were observed napping. Four child records and four staff records were reviewed. The Center has all staff obtain current certification in CPR and First Aid as well as Daily Health Observation Training. Four medications and authorization forms were reviewed and the center has three staff current in Medication Administration Training (MAT). The attendance and emergency drill log were reviewed. The most recent Fire Inspection on file was dated 10/4/21 and the most recent Health Inspection on file was dated 5/17/22. Standard #: 8VAC20-780-160-A-1
Description: Based on review of four staff records, the center did not ensure that each staff member submitted documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with children. Evidence: 1. The TB screening on record for Staff #1, with a hire date of 2/8/22, was dated 3/13/22. 2. The TB screening on record for Staff #2, with a hire date of 2/15/22, was dated 3/12/22. 3. The TB screening on record for Staff #3, with a hire date of 3/14/22, was dated 3/16/22.

Plan of Correction: It has been noted the recent TB change in which all employees must have a TB test before work begins, not up to 21 days after starting work. TB test will be completed before they begin at the center.
