Comments:
A monitoring inspection was conducted on 5/25/2022. There were 59 children in care, with a total of 9 staff members assigned to their direct supervision. The children were observed enjoying an outdoor picnic, listening to quiet music, eating lunch, and taking their afternoon nap. The Inspector reviewed compliance in the areas of the physical plant, staff records, children's records, background checks, staff training, and emergency preparedness.

Violations:
Standard #: 8VAC20-780-130-A
Description: Based on a review of children's health records, it was determined that the center did not obtain documentation that each child received the immunizations required by the State Board of Health before allowing each child to attend the center.
Evidence:
Child B entered care on 11/20/2021. As of the date of this inspection there is no immunization record in this child's file.

Plan of Correction: The parent will be told to bring the shot record or the child will not be able to attend.

Standard #: 8VAC20-780-140-A
Description: Based on a review of children's records, it was determined that the center did not obtain documentation showing that each child had a physical examination under by a physician within 30 days after the first day of attendance.
Evidence:
Child B entered care on 11/20/2021. There is no documentation of a completed physical exam in this child's file.

Plan of Correction: The parent will be told to bring the physical or the child will not be allowed to attend.

Standard #: 8VAC20-780-160-C
Description: Based on a review of staff health records, it was determined that two staff did not obtain a repeat Tuberculosis Health Screening at least every two years from the date of their most recent screenings.
Evidence:
Staff A and Staff B both have expired TB Health Screenings. For each of the,m their most recent screening was completed on 11/19/2019.

Plan of Correction: Both staff have been asked to either bring documentation showing a current TB test or to go get a repeat TB test completed.
