Comments:

A monitoring inspection was initiated on 05/24/2022 and concluded on 05/25/2022. On 05/25/2022, there were 50 children present, with 6 staff supervising. Children and staff were observed while having outside play. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 2 staff records were requested to be reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #:	22VAC40-185-60-A
Description:	Based on record review, children's records did not contain all required information. evidence: 1) On the date of inspection, a record for Child #1 and Child #2 could not be located. 2) The record for Child #3 did not contain telephone numbers for the place of employment of each parent who has custody, and the first date of attendance to the program.
Plan of Correction:	Records for child #2 have been requested from parent. Records for Child #2 have been returned to the school Extended Day office and added to the hard copy file.
Standard #:	8VAC20-780-70
Description:	Based on record review, staff records did not contain all required information. Evidence: 1) The records for Staff #1 and Staff #2 did not contain documentation that a second reference as to character and reputation as well as competency was checked before employment.
Plan of Correction:	Copies of references for staff #1 and Staff #2 were requested from the school office records. copies have been made so that the file of Staff #1 and #2 will have adequate reference documents.
Standard #:	8VAC20-780-550-G
Description:	Based on documentation review and interview with staff, documentation of emergency evacuation, shelter-in-place, and lockdown drills was not maintained. Evidence: 1) On the date of inspection, the inspector requested Staff #3 to provide the emergency drills log so it can be reviewed. Staff #3 could not locate the emergency drills log.
Plan of Correction:	All logs, binders, and files (those taken home the week of May 16-23 for end-of-year organization) have been returned to the school Extended Day office.
Standard #:	8VAC20-820-120-E-2
Description:	Based on review of required posted documents, the center did not ensure that all required documents related to the terms of the license were posted on the premises of the program. Evidence: 1) On the date of inspection, the findings of the most recent inspection of the facility (12/2021) were not posted.
Plan of Correction:	The latest inspection papers (yellow form from 05/24) were posted on our bulletin board in the cafeteria on 05/25 during inspection. Posting printed copy of this completed inspection report to board.
