The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing office on 5/9/2022. Interviews were conducted with staff relating to allegations of staffing and supervision. The preponderance of the evidence gathered during the investigation supports the allegations, so the complaint is determined to be valid. A violation notice was completed. Standard #: 8VAC20-780-340-F Complaint related: No Description: Based on interviews the center failed to ensure that children under 10 years of age always shall be within actual sight and sound supervision of staff. Evidence: The teacher of the 2-3 year old class stepped out of the classroom for approximately one minute, to get some plates for lunch. Plan of Correction: The program director talked to and sent an email to all staff reminding them that observers, trainers or anyone else who is not Pathways E. L. C. staff should not be alone with kids.
Standard #: 8VAC20-780-350-B-3 Complaint related: Yes Description: Based on interviews the center failed to maintain the required staff to children ratios. Evidence: 1. The two year old classroom is allowed a ratio of 1-9 due to the ratio waiver being granted. 2. On May 9, 2022 at 9:00 am the ratio was 1-10. At 10:40 am another child came into care makin the ratio 1-11. The class was out of ratio for approximately one hour. Plan of Correction: The director sent an email to all staff about ratios and that observers or trainers cannot be counted in the ratios.
