Clarified requirements for child assessments conducted by parents and center staff.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and an in person tour of the program.
A complaint inspection was initiated on May 19, 2022 and concluded on June 2, 2022. There were 61 children present, ranging in ages from infants to age 5, with 9 staff supervising. The inspector reviewed compliance in the area of staffing and supervision. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Inspection was amended on June 7, 2022.

Description: Based on observation, record review and staff interviews, the licensee did not ensure the staff-to-children ratio 1 staff or every 8 children for the children age 2 was followed.

Evidence: Between 8:30am and 9:00 am on May 19, 2022 there were 9 children in the two year old classroom, room #106 with 1 staff. Two of the children (child #1 and child #2) on the roll sheet were under 24 months. Child #1 was 23 months old and child #2 was 21 months old

Plan of Correction: Director will ensure moving forward every child will have an assessment prior to being moved up. Parents and center staff completed assessments for child #1 and child #2 and they are in the two year old room. Parents had given written permission for the children to be in the two year old room.

Description: Based on staff interviews, record review and observation the licensee did not ensure when a child is assigned to a different age group it is done with a parent's written permission and a written assessment by the program director and program leader.

Evidence: Child #1, 23 months old and child #2, 21 months old were assigned to the two year old room. Center director confirmed there was not documentation of a written assessment by the program director and program leader conducted for Child #1 and Child #2.
