Comments:
An unannounced renewal inspection was conducted on 6/7/2022 from 11:15am-2:30pm. There were 103 children directly supervised by 17 staff and all classrooms were in ratio. The physical plant, outdoor playground areas, programming, menus, 7 staff records, 10 children?s records, 1 medication, emergency drills, injury reports, emergency supplies, and policies were inspected. Children were observed painting, washing hands, preparing for nap time, having free play, and eating lunch which consisted of cheese pizza, broccoli, fruit, and milk. Positive interactions between children and staff were observed during this inspection. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 22.1-289.035-A
Description: Based on record review, the center did not obtain repeat central registry search results every 5 years as required for all staff.

Evidence: Staff #6's most recent central registry search results on file were completed 4/14/2017.

Plan of Correction: Director sent it after coming back from surgery. Will call to ask.

Standard #: 8VAC20-780-240-A
Description: Based on record review, the center did not obtain documentation of all staff completing the Virginia Department of Education-sponsored orientation course within 90 calendar days of employment.

Evidence: Staff #3 (Date of Hire: 9/16/2021) did not have documentation of completing the Virginia Department of Education-sponsored orientation course.

Plan of Correction: Had staff print out certification of completion.

Standard #: 8VAC20-780-270-A
Description: Based on observation of the physical plant, the center did not ensure that areas and equipment of the center, inside and outside, were maintained a clean, safe, and operable condition.

Evidence:
1. In the Preschool 1 class, an empty wooden shelf approximately 3 feet tall by 2 feet wide used to define classroom center areas was not kept in a way that prevented tipping. A child was observed pulling on the top of the shelf which resulted in the shelf tipping over on top of the child and knocking the child to the ground. Staff had to lift the shelf off the child.
2. In the bathroom in the Kindergarten room, one of the toilets' seats was not attached to the base and was turned to the side with the bolt attachment laying on the floor below.
3. In the bathroom in the Kindergarten room, there was a section of cove base missing revealing a hole in the drywall by the floor.
4. In the Jr. Preschool room, the blue cushion on the couch in the center of the room had tears at two corners revealing the foam cushioning.
5. In the Toddlers room, there were two screws protruding from the walls. The first screw protruding was located above a cot positioned below the window where children were preparing to nap. The second screw protruding was close to the ground below the "Kind Child" board.

Plan of Correction: 1. Shelf was removed until it can be mounted.
2. Maintenance was scheduled to come out 6/9/22 to fix the toilet.
3. Temporary base board was placed.
4. Blue cushion was replaced.
5. Boards and screws were taken off the walls immediately. Holes were patched and painted.
