The licensing inspector conducted an unannounced complaint inspection on 06/07/2022 in response to a complaint that was received in the licensing regarding allegations in the area of supervision Interviews were conducted with staff, records reviewed and observations of the program were made relating to the allegation. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22VAC40-185-340-A Complaint related: No Description: Based on record review and interviews, the center did not ensure the care, protection, and guidance of children when supervising them.
Evidence:
On 06/06/2022 around or about 12:30pm Staff A sent 5 year-old Child 1 to the office unsupervised, to gather papers from the copy machine. At around the same time a prospective parent rang the bell to the center. Child 1 opened the door and asked the parent, "Can I Help You?" The parent asked for an adult and child closed the door.Plan of Correction: The following corrective action was provided by the district manager:
Corrected on 06/08/2022. Staff was retrained on child supervision to ensure all children are within sight and sound while in care. All staff are trained during orientation on proper supervision of children prior to being in ratio. Management will do periodic inspections and ensure a manager is available throughout the day if staff need assistance.
