Children were observed interacting with staff and participating in free play. A sample of 2 children's records and 2 staff records were reviewed. There were no injury reports or medications for review. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

2. Staff confirmed that this documentation was not available during this inspection.
Plan of Correction: Staff will re-request documentation of a physical.
