A monitoring inspection was conducted on 6/14/22 from 9:40am until 11:30am. At the time of the inspection, there were 42 children in care with 8 staff present. A sample of 4 children's records and 3 staff records were reviewed. Children were observed playing on the playground, eating snack, participating in morning learning activities and resting quietly during nap time. First aid and emergency supplies, the emergency preparedness plan, documentation of emergency practice drills and required center postings were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law. Violations are listed on the violation notice issued to the center and were discussed with the Head of School during the exit interview.

Standard #: 8VAC20-780-260-B Description: Based on a review of the most recent health inspection and interview, the center did not ensure that annual approval from the health department shall be provided.
Evidence:
1. The most recent health inspection for the center is dated 5/20/21. An updated health inspection was due in May 2022.
2. The Head of School confirmed that the health inspection is expired.Plan of Correction: The center responded with the following: Center management will contact the health department to schedule the annual health inspection as soon as possible.
Standard #: 8VAC20-780-280-B Description: Based on observation and interview, the center did not ensure that hazardous substances such as cleaning materials, insecticides and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. In the school-age classroom, there was a can of wasp and hornet spray, a can of disinfectant spray, a bottle of Goo Gone, a container of sanitizer tablets, a container of antibacterial cleaner and a container of Lysol wipes in an unlocked cabinet over the sink.
2. The Head of School confirmed that there were unlocked hazardous chemicals in the school-age classroom.Plan of Correction: The center responded with the following: A lock will be placed on the cabinet containing hazardous substances in the school-aged classroom.
Standard #: 8VAC20-780-280-G Description: Based on observation and interview, the center did not ensure that if hazardous substances are not kept in original containers, the substitute containers shall clearly indicate their contents.
Evidence:
1. In the school-aged classroom, there was an unlabeled spray bottle containing a cleaning material.
2. The Head of School confirmed that the unlabeled spray bottle contained a cleaning material.Plan of Correction: The center responded with the following: Going forward, the center will ensure that substitute containers will be labeled to clearly indicate their contents.
Standard #: 8VAC20-780-560-G Description: Based on observation and interview, the center did not ensure that when food is brought from home the food container shall be clearly dated and labeled in a way that identifies the owner.
Evidence:
1. None of the forty-two children's lunchboxes were observed to be dated.
2. The Head of School confirmed that the children's lunchboxes were not dated.Plan of Correction: The center responded with the following: Center management will remind staff and parents to date the lunchboxes each day. An allowable variance will be requested for this standard, as children take their lunchboxes home at the end of each day and any leftover food is disposed of.
