Technical Assistance:
Technical assistance was given regarding the new standards, outdoor play area, fence, staff personal items, topical products and MAT certification.

Comments:
An unannounced renewal inspection was conducted today from 9:35am-11:20am. There were 36 children (infants-4yrs.) directly supervised by 7 staff. The physical plant, 3 staff records, 3 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. There were no medications on-site during the inspection. Children were observed playing on the playground. There was an abundant supply of materials for the children. Staff-to-child ratios were in compliance. The center was clean and organized.

Standard #: 22.1-289.036-A
Description: Based on record review and interview, a central registry check for an agent was not completed every 5 years.

Evidence:
Agent #1's last Central Registry search result was dated 5/3/2016.

Plan of Correction: Results have been emailed to inspector.

Standard #: 8VAC20-780-160-A
Description: Based on record review, the center did not obtain documentation of a negative tuberculosis screening for each staff member.

Evidence:
Staff #1's record does not contain documentation of a negative tuberculosis screening/test. The staff member was hired on 12/27/2021.

Plan of Correction: Teacher got test done and results coming. Teachers records with health department, but not in center.

Standard #: 8VAC20-780-245-L
Description: Based on record review and interview, there was not at least one staff member on present during the inspection who had obtained within the last 3 years instruction in performing the daily health observation of children.

Evidence:
There was not one staff member on duty with a current Daily Health Observation certification.

Plan of Correction: Three teachers enrolled in daily health observation trainings.

Standard #: 8VAC20-780-280-B
Description: Based on observation cleaning materials were not kept in a locked place using a safe locking method that prevents access by children.

Evidence:
In the 4 year old classroom, a container of Clorox disinfectant wipes and a spray can of Lysol deodorizer were observed on a shelf under the classroom's calendar, unlocked and accessible to children.

Plan of Correction: Boxes with lock and key were bought for every class.

Standard #: 8VAC20-780-530-A
Description: Based on record review and observation, at least one staff member with current cardiopulmonary resuscitation (CPR) and first aid (F/A) training certification was not present in each classroom or area where children are present.

Evidence:
Staff #1 was observed alone with 3 children in the infant classroom, the staff member's file did not contain CPR/FA training certifications.

Plan of Correction: Teacher will receive CPR training on July 5, 2022.

Standard #: 8VAC20-780-540-A
Description: Based on review of emergency supplies, the first aid kit was incomplete.

Evidence:
The first aid kit was missing 1 of 2 required triangular bandages.

Plan of Correction: Triangular bandage bought.
