An onsite unannounced monitoring was initiated and concluded on May 27, 2022. There were 45 children in care with
nine staff. A sample of children and staff records were reviewed. Children were observed in teacher and self-directed activities including but not limited to snack, outside play and restroom routines. Annual inspections, emergency evacuation drills, required postings, first aid kit, emergency equipment and medication were reviewed. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice and discussed with the center representative during the exit interview.

Standard #: 8VAC20-780-70 Description: Based on record review and interview the center failed to maintain staff records as required.
Evidence:
1 - The records for staff 4 and 5 do not include documentation of education.
a. Staff 4 has a documented date of hire as September 7, 2021. Staff 4 was hired as Program Leader but there is not any documentation on file to support this job title. Staff 4 was present working with children during the inspection
b. Staff 5 has a documented date of hire as September 7, 2021. Staff 5 was working in the position of program leader during the inspection. Standard #: 8VAC20-780-350-B-4 Description: Based on observation the center failed to ensure that that required ratios are followed.
Evidence:
1 - During the inspection in the four/five-year-old room there were two groups of children 12 children with staff 2 in one area and 11 children with staff 3 in another area of double wide classroom. There were a total of 23 children in care with two staff.
2 - Staff 2 and 3 report that they have been operating with 1 staff to 12 children ratio during the entire school year.
3 - Staff 1 confirmed that the center had run a 1:12 ratio with the preschool group.Plan of Correction: Not available online. Contact Inspector for more information.
