An unannounced inspection was conducted on 6/13/2022.
Began 11:45 am ended 2:20 pm.
Total of 78 children present with 14 staff.
Upon entering the center the classes were finishing lunch and preparing for nap.
A review of five children and five staff records was conducted.

Standard #: 22.1-289.035-B-4 Description: Based on record review the center failed to ensure that all persons who indicate on a sworn statement having lived outside Virginia in the last five years, and who work alone with children, have completed all required background checks prior to employment.
Evidence:
There were no out-of-state central registry background checks sent for staff staff #1 (hire date 3/9/22) and staff #4 (hire date 10/19/21), as of the date of the inspection.Plan of Correction: out of states not needed NF states [sic]

Standard #: 8VAC20-780-60-A Description: Based on record review the center failed to ensure that all the required information was obtained on each child enrolled.
Evidence:
1. Child #3 & # 4 does not have a physical on file and both have been in care over 30 days.
2. Child #3 does not have an immunization record on file.Plan of Correction: Both children had shot records in email they are printed & in file. [sic]
Standard #: 8VAC20-780-280-B Description: Based on observation the center failed to ensure that hazardous substances such as cleaning material, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. In the three year old class located on a shelf and not locked was Lysol, VaVoom, window cleaner and a cleaning agent.
2. In the school age area is a wooden cabinet, not locked. The contents included hard surface sanitizer, window cleaner, furniture spray, disinfectant deodorizer, two bottles of liquids not labeled.
3. In the storage room between the school age room and the four year old rooms was not locked. The contents included: Kilz, disinfectant spray, spray paint, rubbing alcohol, glass cleaner, floor cleaner, hard surface sanitizer, mop bucket with water, bleach, and carpet shampooPlan of Correction: 3 yr old cabinet not reachable by children

Standard #: 8VAC20-780-410-1 Description: Based on observation the center failed to ensure that the following actions or threats thereof are forbidden: roughly handling a child.
Evidence:
While observing the four year old class during nap time the LI witnessed a child squirming around on his mat and pulling his legs up under him while lying on his stomach. Staff #1 on two occasions grabbed the child's legs and yanked them so they would be straight and then got very close to his face and said "lay down!", forcefully.Plan of Correction: Training For Staff # 1 and all center staff. [sic]
Standard #: 8VAC20-780-440-E Description: Based on observation the center failed to ensure that there shall be at least 12 inches of space between occupied cots.
Evidence:
In the three and four year old classrooms several of the occupied cots had four to six inches of space between them.Plan of Correction: With the way shelves are in rooms cots couldn't be 12 inches. Scooted shelves to spread out cots more [sic]
