An unannounced monitoring visit and facility tour was conducted on 6/13/2022. At the time of inspectors' arrival there were 40 toddler to preschool aged children in care with 4 staff members and 1 administrator. Children were observed interacting with staff, participating in educational activities, outdoor play, lunch and hand washing. A sample of 5 children's records and 5 staff records were reviewed. There were no Injury reports or medications to be reviewed. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Standard #: 8VAC20-780-140-A Description: Based on record review it was determined the center did not ensure each child had a physical examination by or under the direction of a physician within the required timeframe.
Evidence:
The records for child #2 and #3 did not contain documentation of physical exams. Child #2 and #3 were in care during this inspection.Plan of Correction: Director will request documentation of physical exams from the parents.
Standard #: 8VAC20-780-270-A Description: Based on observation it was determined the center did not ensure all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition.
Evidence:
1. There was broken plaster around the electrical outlet and light switch in the toddler classroom.
2. There was exposed drywall in the two year old classroom.Plan of Correction: Center will repair walls.
Standard #: 8VAC20-780-280-B Description: Based on observation it was determined the center did not ensure that all hazardous substances such as cleaning materials, insecticides, and pesticides were kept in a locked place using a safe locking method that prevents access by children.
Evidence:
There were two spray bottles of cleaner/sanitizer on top of a shelf in the three year old classroom, where children were in care.Plan of Correction: Center will make sure all cleaners and chemicals are locked and inaccessible to children in care.
