22.1 Background Checks Code; carbon monoxide detectors

An unannounced monitoring inspection was conducted on June 24, 2022. At the time of entrance there were 12 children and 5 staff members present with children. The sample size consisted of 2 children's records and 2 staff records. Children were observed outdoors, circle time and participating in tabel activites. . The inperson inspection began at approximately 9:55 am and ended at approximately 11:45 am. Areas of non-compliance are identified on the violation notice.

Standard #: 8VAC20-770-60-C-2 Description: Based on record review and staff interview, the licensee did not ensure a central registry finding were recieved within 30 days of employment.
Evidence:
Staff #1 ( date of employment 03/07/2022)record did not have central registry results with in 30days. Confirmed by Staff #4.Plan of Correction: The center contacted the Central Registry office on the day of the inspection. The center will try to contact Central Registry office again. If they do not get a respose from the offices the center will have another Central Registry processed by staff.
Standard #: 8VAC20-780-70 Description: Based on record reveiw, the licensee did not ensure staff records obstained all required documents.
Evidence:
The record for staff #1 and staff #2 were missing documentation to demonstrate that the individual possesses the education and certification job position.Plan of Correction: The center will have all information on file with certification and education for job position.
Standard #: 8VAC20-780-240-A Description: Based on record review and staff interview, the licensee did not ensure staff had The Virginia Department of Education-sponsored orientation course completed within 90 calendar days of employment.
Evidence:
Staff #1( date of employment 03/07/2022 ) has not completed Va Department of education-sponsored orientation with in 90 days of employment. Confirmed by Staff #4.Plan of Correction: Staff will have the Va Preservice training completed by July 6, 2022.
Standard #: 8VAC20-780-270-A Description: Based on observation , the licensee did not ensure all outdoor equipment was in a safe and operable condition.
Evidence: The outdoor area had a broken wheel on the radio flyer which is cracked could cause snagging or pinching. The little playhouse outside door handle was broken. There was trike that was missing rubber handle covers which leaves a metal handle exposed.Plan of Correction: The center will fix the door handle on the playhouse . It was already locked from the inside of the building.
The center contacted a company to get covers for the bike handles.
The other piece of equipment was thrown away.
Standard #: 8VAC20-780-500-B Description: Based on observation, the diapering surface was not nonabsorbent.
Evidence:
The diaper pad in the bathroom had two tears, making it a nonabsorbent surface.Plan of Correction: We use a barrier over the diapering surface. The diapering pad will be replaced by the 5th of July.
Standard #: 8VAC20-780-530-C Description: Based on observation and staff interview, the licensee did not ensure there was at least two staff members that aew CPR and First Aid present on the premises during the center's hours of operations.
Evidence:
There was not two people with CPR and First Aid certified at the center upon arrival. Staff #4 arrived approximatley 10 mins later . Staff #4 stated there is a certification setup for all staff in mid July.Plan of Correction: We have two staff with CPR and First AId. We have a training schedule for July 17th, 2022 for all staff. We are trying to schedule to be taught earlier.
Standard #: 8VAC20-820-120-E-6 Description: Based on observation and inspection of the facility, the licensee did not ensure that a copy of any special order issued by the department was posted in a prominent place at each public entrance of the licensed premises to advise consumers of serious or persistent violations.
Evidence: Staff #4 confirmed that the special order issued by the department was not posted in the facility.Plan of Correction: The center director did not recieve the copy of the Special Order . The owner stated previously documents were emailed to the Director. The owner was not aware the document was not sent to the director. The owner had emergency which made him unavailable to check on the postings. The information is posted in the facility.
