Comments:
6/8/22- An in person complaint investigation was begun today in response to an allegation being received in our office. The investigation is on-going at this time.
6/10/22 - Based on staff interviews, the preponderance of evidence supports the allegation of forbidden actions. The complaint is valid. An additional violation cited was not part of the original complaint but found during the course of the investigation.

Description: Based on observation, interview and a review of documentation the center did not ensure that advertising is not false and misleading.
Evidence:
The facility's application for licensure states the hours of operation for the preschool are 9:00 AM- 3:30 PM with that information displayed on the DOE website. A review of records and an interview revealed at least one preschool child is attending daily from 7:00 AM - 5:30 PM. (Child 1: Date of Birth: 4/24/2018.

Description: Based on staff statement, a staff member used a forbidden action: restricting a child's movement.
Evidence: A staff member "had to hold a child down" when the child had an alteration regarding a toy. (Staff A)
