Comments:
A monitoring inspection was initiated on May 2, 2022 and concluded on May 3, 2022. There were 28 children present, ranging in ages from Infants to preschool, with nine staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and five staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-160-C
Description: Based on a review of staff records, the center did not ensure that subsequent communicable tuberculosis (TB) screenings were conducted at least every two years from the date of the initial screening.

Evidence:
The record for staff #4 (date of last TB screening 05/03/19) did not have an updated TB. screening/test

Standard #: 8VAC20-780-40-E
Description: Based on review of policies and procedures, observation and interviews, the center failed to ensure that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.

Evidence:
1. In class #2 and class #4 children were napping on cots without the supervision of a staff member.
2. The centers policies state that a staff member's job description includes ?Makes sure children are supervised when using any of the toys and equipment? and ?leaving children unattended for any reason any length of time will be grounds for immediate termination".

Standard #: 8VAC20-780-340-F
Description: Based on observation and interview the center failed to ensure that children under 10 years of age always are within actual sight and sound supervision of staff.

Evidence:
1. In class #2, there were seven toddlers napping alone in the classroom when the inspector entered the classroom. The inspector did not observe any staff supervising the children in class #2.
2.In class #4, there were eight two year old's napping alone in the classroom when the inspector entered the classroom. Staff A stated, ?I just ran up there for a second? when returning to class #4 after approximately one minute.
